Job Summary
RN Case Manager in hospice delivering coordinated care to homebound patients. Responsibilities include delivering hospice nursing care, conducting thorough assessments, planning and implementing effective care, managing pain and symptoms, ensuring patient safety, educating patients and families on disease processes and end-of-life care, and producing timely documentation. Works with an interdisciplinary hospice team in community or home settings; emphasizes compassionate, patient- and family-centered support and adherence to Bloom Healthcare standards.
Required Qualifications
- Accredited nursing school graduate
- Valid CO RN license
- At least one year in med-surg, emergency or critical care nursing
