Hospice Registered Nurse Case Manager

Job Summary
Conduct comprehensive assessments of patients and families to determine hospice needs, then initiate and regularly revise care plans based on evolving status. Administer prescribed medications, counsel patients and caregivers on self-care techniques, and facilitate activities of daily living while coordinating with physicians and community health resources. Act as Case Manager when assigned to coordinate patient care for the caseload, attend interdisciplinary meetings, and participate in on-call duties as defined by policy. Maintain accurate clinical notes, report abuse or neglect immediately, and ensure equipment and ancillary personnel support are available.
Required Qualifications
- Graduate of an accredited school of nursing
- Registered Nurse, with a License in good standing to practice
- Minimum of one (1) year of experience as a professional nurse within the last three (3) years
- Baccalaureate degree in nursing
- Minimum of two (2) years' experience
- At least one of which is in the area of public health, home care, or hospice nursing
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Visual acuity and hearing to perform required nursing skills
- Must be a licensed driver
- Automobile that is insured in accordance with state/or organization requirements
- Automobile is in good working order
- Possesses and maintains CPR certification
- CPR certification demonstration of skills was hands on and observed by a certified trainer
