Hospice Registered Nurse Case Manager

Job Summary
Conduct comprehensive assessments of patients and families to determine hospice needs and initiate plans of care. Administer medications and treatments as prescribed, while counseling patients and caregivers on self-care techniques and meeting emotional needs. Coordinate patient care for assigned caseloads by communicating with physicians and community health resources, and attend interdisciplinary group meetings. Complete accurate clinical notes, report suspected abuse or neglect to appropriate agencies, and participate in on-call duties. Ensure equipment availability and supervise ancillary personnel as required.
Required Qualifications
- Graduate of an accredited school of nursing
- Registered Nurse, with a License in good standing to practice
- Minimum of one (1) year of experience as a professional nurse within the last three (3) years
- Baccalaureate degree in nursing
- Minimum of two (2) years' experience
- At least one of which is in the area of public health, home care, or hospice nursing
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Visual acuity and hearing to perform required nursing skills
- Must be a licensed driver
- Automobile that is insured in accordance with state/or organization requirements
- Automobile is in good working order
- Possesses and maintains CPR certification
- CPR certification demonstration of skills was hands on and observed by a certified trainer
