Job Summary
Conduct daily field visits to provide emotional support and implement individualized care plans for patients in their familiar surroundings. Perform initial and ongoing assessments of physical, functional, psychosocial, and environmental needs while consulting with families on disease processes and end-of-life care. Supervise paraprofessionals, prepare clinical progress notes, and participate in interdisciplinary group meetings and performance improvement programs. Maintain compliance with Medicare policies and AdvisaCare procedures while adapting to patient status changes on a full-time on-call basis. Required Qualifications
- Current Michigan License in good standing
- Minimum 1 year hospice experience
- Empathetic approach to patients and caregivers
- Ability to adapt to patient's status and needs on a full time on call basis
