Job Summary
Perform initial and ongoing comprehensive nursing assessments for hospice patients and develop individualized plans of care to ensure continuity. Coordinate patient care with the Interdisciplinary Team, physicians, and families while educating them on disease processes and symptom management. Supervise Hospice Aides and LVN/LPN staff, maintain accurate clinical documentation, and monitor assigned cases for regulatory compliance. This role requires a Registered Nurse with hospice experience, working on-call in the Grayson County/Denison/Gainesville territory.
Required Qualifications
- Registered Nurse (RN)
- Licensed per state requirements
- Reliable transportation
- Valid driver's license
- Current auto liability insurance
- Ability to read, write, and communicate effectively in English
