Job Summary
Perform initial and ongoing comprehensive nursing assessments for hospice patients in their place of residence. Develop, implement, and update individualized hospice plans of care while coordinating patient care with the Interdisciplinary Team, physicians, and families. Educate patients and families on disease process, medications, and symptom management. Supervise Hospice Aides and LVN/LPN staff, maintain accurate clinical documentation in the electronic medical record, and ensure compliance with Medicare regulations and agency policies. Monitor assigned cases to ensure third-party payer compliance and participate in case conferences and IDT meetings. This role covers the Grayson County/Denison/Gainesville territory and requires a licensed RN with hospice experience.

- Registered Nurse (RN)
- Licensed per state requirements
- Reliable transportation
- Valid driver's license
- Current auto liability insurance
- Ability to read, write, and communicate effectively in English
