Hospice PRN Registered Nurse (RN)

Oversee end-of-life care for patients in their homes, evaluating treatment conditions and individualizing plans of care as needs change. Monitor patients to ensure care levels meet requirements, update doctor orders, and administer medications per physician directives. Counsel families on disease progression while incorporating them into care planning and manage multi-disciplinary admissions, transfers, and discharges. Supervise nurse aides and LPNs, complete clinical documentation per agency and Medicare guidelines, and attend mandatory training.
