Hospice PRN Registered Nurse Case Manager (CM)

Job Summary
Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs to determine admission eligibility and certify care levels. Implement and document individualized plans of care while consulting with the interdisciplinary team, families, and referring agencies. Educate patients and families on disease processes, self-care techniques, nutrition, and dietary requirements. Provide clinical directions to aides and LPNs/LVNs, monitor case compliance with third-party payors, and prepare progress notes demonstrating goals. Perform on-call services for emergency calls and participate in QAPI programs and in-service training. Requires current RN licensure, two years of clinical experience, and ability to work flexible hours in patient homes.
Required Qualifications
- Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current state license as a Registered Nurse (RN)
- Current driver's license
- Two (2) years experience as a registered nurse in a clinical care setting
- Must read, write and comprehend English
- Reliable transportation
- Valid auto liability insurance
- Ability to work a flexible schedule
- Ability to travel locally
- Prolonged standing and walking
- Ability to lift up to 50 lbs and move patients
- Ability to work under some stressful conditions
- Ability to make quick decisions and resource acquisition
- Eye-hand coordination and manual dexterity
- Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs
- Apply specific criteria for admission and re-certification to hospice care
- Implement/develop/documents the plan of care
- Consults with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, Nutrition, and dietary needs
- Prioritize any needs with the members of the IDG/T
- Provide training to other staff as needed
- Determine scope and frequency of services needed based on acuity and patient/family needs
- Assess the ability of the care giver to meet the patient's immediate needs upon admission and throughout care
- Initiate appropriate preventive and rehabilitative nursing procedures
- Apply concepts of infection control and standard precautions
- Provide clinical directions to the Hospice Aide and LPN/LVN
- Prepare clinical and progress notes
- Inform physician and other personnel of changes in the patient's needs and outcomes of intervention
- Monitor assigned cases to ensure compliance with requirements of third party payors
- Participate in the Hospice sponsored in-service programs
- Maintain or improve competency
- Participate in the Hospice's QAPI program
- Promote the Hospice's philosophy and administrative policies
- Perform on-call responsibilities and provide on-call services to patients and their families as assigned
- Provide effective communication to patients, family, team members, and other health care professionals
