Job Summary
Respond to after-hours patient and family calls with professionalism and compassion, assessing needs and providing symptom management or end-of-life support. Complete urgent visits in patients' homes or facilities as needed, communicating status changes to the clinical team and physicians while educating caregivers. Document all interactions timely and accurately in the EMR system. Requires an active Texas RN license, CPR certification, and reliable transportation for travel throughout the service area. Competitive on-call compensation, visit pay, and mileage reimbursement are available.
Required Qualifications
- Active Texas RN License
- Current CPR certification
- Reliable transportation
- Willingness to travel throughout the service area

- Hospice, home health, long-term care, or acute care experience
- Hospice experience
