Job Summary
Oversee medical direction and treatment for hospice and palliative care teams, conducting daily rounds at the hospice house and making home visits for difficult cases. Provide 24-hour coverage via on-call rotation to evaluate patient eligibility, answer after-hours calls, and support clinical teams in the field. Collaborate with interdisciplinary teams and primary care physicians to develop care plans, certify medical prognosis, and manage discharge or decertification. Execute in-service programs, participate in weekly team meetings and QAPI committees, and conduct educational seminars for healthcare professionals. Maintain medical staff privileges and resolve clinical or ethical conflicts as assigned.
Required Qualifications
- Current license to practice medicine in New York State
- Graduate of an accredited school of medicine and training program
- Minimum of five years' experience as a primary care physician, internist, oncologist or other medical specialty identified as appropriate by UH
- Current DEA registration/license
- Certification or eligibility for certification by the American Board of Hospice and Palliative Medicine or comparable program
- Possesses working knowledge of PC based applications
- Must have or be eligible for membership in good standing on medical staff of area hospitals
- Maintain medical staff privileges at a hospital which Hospice has a contract
- Qualifies for insurance coverage under Hospice's professional liability policy
