Job Summary
Certify eligible patients for hospice services, order medications and equipment, and document all visits and communications in clinical records. Coordinate care with attending physicians, lead the interdisciplinary team, and monitor patient status through group meetings to modify care plans. Participate in on-call rotations for patient admissions, liaise with hospital staff, and certify death certificates as needed. Provide education to the interdisciplinary and hospice teams, conduct chart audits, and maintain medical staff credentials while adhering to legal standards and Calvary's Code of Conduct. Full-time role with a salary of $250K plus bonus opportunity.
Required Qualifications
- Current New York State license
- DEA registration
- Possess a valid driver's license
- Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions
