Job Summary
Conduct compassionate, field-based care for patients and families in the Zanesville service region, traveling to homes and facilities within the greater Dayton/Trotwood area. Deliver comfort and dignity while documenting patient services in electronic medical records using proficient computer skills. This full-time position involves 4-10 hour shifts from 8am to 6pm and requires a valid Ohio driver's license with proof of auto insurance. Candidates must pass a criminal background check and 10-panel drug screening, with CHPNA certification preferred. Mileage reimbursement and company-provided technology are included, alongside opportunities for internal training toward CHPNA certification. Required Qualifications
- High School Diploma or GED
- Proficient computer skills with the ability to accurately document patient care and services in electronic medical records
- Valid Ohio driver's license with a safe driving record and proof of auto insurance
- Ability to pass a criminal background check and 10 panel drug screening
Desired Qualifications
- Certified Nurse Aide (preferred, but will train internally)
- Certified Hospice and Palliative Nurse Aide (CHPNA) certification preferred
