Job Summary
Hospice Patient Care Manager to oversee clinical field staff and coordinate patient care, ensuring timely admissions, supervision and development of staff, compliance with regulations, and collaboration with the Clinical Director. Responsibilities include directing patient care plans, coordinating interdisciplinary team meetings, managing staffing and performance evaluations, maintaining accurate records, and participating in budget and regulatory compliance. Strong leadership, nursing expertise, and ability to supervise nurses and aides, plus coursework or experience in healthcare management and community engagement. Travel locally may be required; on-site in Wyomissing, PA.
Required Qualifications
- PA Registered Nurse License (Required)
- Bachelor’s degree in nursing (Required)
- High school Diploma or GED (Required)
- Valid PA Driver's License
- CPR Certification (Required)
- Completion of Highlands Hearthstone Ambassador training within 30 days of employment (Required)
- Must be a licensed driver with automobile
- At least one year of management experience in a hospice or health care organization (Preferred)
