Job Summary
Respond to on-call services and uphold nursing service standards throughout the organization by making visits to home or facility patients. Conduct scheduled and unplanned visits, including follow-ups on new admissions, admission visits for medically eligible patients, and facility-to-home transfers. Demonstrate excellence in nursing practice for assignments while ensuring compliance with regulatory and accreditation standards. Requires current RN licensure in Kentucky or Indiana, a valid driver's license, and knowledge of palliative care, with a Monday through Wednesday 5:00 PM to 1:30 AM schedule and backup on-call coverage.
Required Qualifications
- RN with current RN licensure in the state of Kentucky and/or Indiana as required by designated position, or the ability to obtain required license upon hire
- Minimum of 1 year of nursing experience
- Knowledge of palliative care
- Must have a valid driver's license in accordance to policy
Desired Qualifications
- BSN preferred
- CPHN encouraged
- previous home health or hospice experience preferred
- Use of technology-based tools, including electronic medical record, strongly preferred
