Job Summary
Complete General Inpatient Admissions and conduct daily patient visits while transitioning patients from Emory Hospital to home hospice care. Collaborate with physicians and mid-levels to determine eligibility for hospice, implement individual care plans, and educate patients on home services. Participate in biweekly meetings to review patient progress. Requires a Registered Nurse in Georgia with two years of patient care experience and one year in hospice or palliative care. Schedule includes a 7 on 7 off rotation from 7pm to 7am.
