Hospice Nurse - RN (PRN)

Collaborate with physicians to oversee and implement personalized care plans while performing initial and comprehensive assessments on all patients in accordance with policies and procedures. Provide consistent, high-quality patient care to ensure an exceptional experience, ensuring clear, concise, and accurate clinical documentation. Assess the need for plan of care updates while keeping the patient and family involved in care planning decisions. Work across home, long-term care, assisted living, and hospital settings to address physical, emotional, and spiritual needs.
