Hospice Nurse-RN On Call

Triage after-hours patient and family phone calls, determining needs for assessments, symptom management, and care plan updates. Obtain data on physical, psychological, social, and spiritual factors impacting patient needs, initiating interdisciplinary team interventions as appropriate. Initiate communication with attending physicians and other agencies to coordinate optimal care. Perform clinical pronouncements of death and provide family support. Maintain accurate clinical records through timely preparation of care plans and progress notes. Requires current RN license, two years of varied experience, CPR certification, and a valid driver's license. Supports Broadview Heights and surrounding counties on a 7 on 7 off schedule.
