Job Summary
Hospice Nurse Practitioner provides direct hospice clinical care including assessments, symptom management, treatment planning, and follow-up in IPU and home-based settings. Partners with Medical Director, hospice physicians, and interdisciplinary team to coordinate services, educate patients and families, ensure documentation and regulatory compliance, and support care planning across settings. Responsibilities include face-to-face visits, eligibility evaluations, patient and family education, and collaboration with the IDT. Requires Wisconsin NP license and master's in nursing; schedule Monday–Friday ~8:00am–4:30pm with on-call coverage and periodic weekend coverage; territory includes Manitowoc, Sheboygan, and Ozaukee Counties.
Required Qualifications
- Current Wisconsin Nurse Practitioner license
- Master’s degree in nursing
- Strong clinical judgment, communication, and interpersonal skills
- Professionalism, tact, and sound decision-making
- Clinical expertise in medications, sterile techniques, and safety practices (PPE)
- Effective leader and educator with collaboration and independent work capabilities
- Proficient in computer use and clinical documentation
- Self-directed, adaptable, and able to manage stress and emergency situations
- Meets driving requirements, including valid license, reliable transportation, and insurance
