Job Summary

Conduct face-to-face visits for initial and ongoing hospice eligibility, assessing disease-related changes, functional status, cognitive decline, and symptom burden. Communicate significant findings, including unmanaged symptoms and eligibility concerns, to the designated hospice physician. Engage with the interdisciplinary team to provide symptom management recommendations and prescribe medications as necessary. Document visits in an electronic medical record in a timely fashion. Monday through Friday, 9 am to 5 pm (40 hours). Master's degree and current Massachusetts NP license required with two years of nursing experience, including one year in hospice, palliative care, or oncology. Familiarity with Medicare Part A regulations and proficiency in electronic documentation are essential.
