Hospice Nurse Practitioner - PRN

Job Summary
Nurse Practitioner (Hospice) to provide in-home hospice care, conduct eligibility assessments, perform comprehensive patient evaluations, develop individualized hospice care plans, collaborate with the interdisciplinary team, prescribe treatments, educate patients and caregivers on symptom management and care planning, participate in quality improvement and compliance initiatives, and maintain accurate clinical documentation. Requires APRN licensure in the state, Master’s in Nursing, hospice/geriatrics certification preferred, minimum 1 year of clinical experience in hospice/home health or similar, BLS/CPR, valid driver's license, and proficiency with mobile tech and electronic charting.
Required Qualifications
- Licensed APRN in the state of employment
- Master's degree in Nursing from an accredited NP program
- BLS/CPR certification
- Valid driver's license, auto insurance, and reliable transportation
- Comfortable using mobile technology and electronic charting systems (Homecare Homebase, Microsoft 365)
- Ability to work independently and respond urgently to patient needs
