Job Summary
Hospice Nurse Practitioner (PRN) provides palliative and hospice care, performing routine and emergency assessments, managing symptoms, and coordinating with the interdisciplinary team to support patients and families in home-based settings. Responsibilities include documenting observations and treatments, ordering and interpreting tests within scope, prescribing medications including controlled substances, educating patients and caregivers on care, disease process, and prognosis, and ensuring compliance with practice standards. The role may involve on-call coverage, after-hours visits, and active collaboration with Medical Director, Attending Physician, DCS, and IDT to deliver high-quality end-of-life and palliative care. Requires Master’s degree in Nursing, NP licensure in the state of employment, active NPI, and collaborative practice where applicable; board certification in Hospice and Palliative Care is preferred but not required. Adheres to confidentiality, regulatory guidelines, and organizational culture, and supports care coordination for referrals. The position is in a PRN, part-time capacity across home-based hospice/palliative services, with locations in Mansfield and Marion, Ohio.
Required Qualifications
- Master's degree in Nursing
- Active and unencumbered Nurse Practitioner license in state of employment
- Board certification in Hospice and Palliative Care preferred but not required
- Active and unencumbered NP license in state of employment required
- Current collaborative physician agreement in place (All states except Iowa)
