Hospice Nurse Practitioner - PPV

Job Summary
Conduct routine and emergency assessments for hospice and palliative care patients, including recertification visits, admissions, and transfers. Provide bedside palliative care, manage acute and chronic conditions, and order laboratory or radiology tests within scope. Prescribe medications, order durable medical equipment, and coordinate referrals with physicians and the interdisciplinary team. Educate patients and families on disease processes, symptom control, and advance care planning. Maintain accurate patient records, complete billing and coding per CMS guidelines, and submit clinical documentation within 24 hours of visits. Flex schedule to meet agency needs, including nights, weekends, and after-hours emergency visits.
Required Qualifications
- Master's degree in Nursing
- Active and unencumbered CNS or Nurse Practitioner license in state of employment
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
- Active NPI number
- Active DEA certification if state allows prescription of controlled substances by APPs
- Current collaborative physician agreement in place
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
- Must be able to work a flexible schedule to include nights and weekends
- Must be able to travel
- Ability to maintain confidentiality of information, such as patient and Company files
- Minimum of one (1) year nursing practice or equivalent experience
- Minimum of five (5) years nursing experience
- Clinical nursing care in oncology, hospice, geriatrics, intensive care
- Ability to triage consults by patient acuity
- Ability to maintain active DEA certification if state allows prescription of controlled substances by APPs
