Job Summary
Provide comprehensive hospice services by evaluating health histories, diagnosing conditions, and developing individualized plans of care under physician supervision. Conduct face-to-face patient assessments, order diagnostic tests, and prescribe medications while managing terminal illnesses and symptom control. Collaborate with interdisciplinary teams to coordinate care, review eligibility for hospice certification, and establish referrals. Educate patients and families on preventative, medical, and spiritual needs while maintaining accurate clinical documentation and compliance with Medicare and HIPAA regulations.
Required Qualifications
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program
- Current DEA license, if applicable to state regulations
- Current nursing licensure in State
- CPR certification
- Master's degree
- Minimum of one (1) year Nurse Practitioner experience
- Minimum of one (1) year hospice or palliative care experience
- Accreditation from an approved certifying body for advanced practice nursing as required by State
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Prolonged or considerable walking or standing
- Able to lift, position and / or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling and/or crouching
- Visual acuity and hearing to perform required nursing skills
Desired Qualifications
- Certification in a specialist area (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
