Hospice Nurse Practitioner

Job Summary
Conduct Face-to-Face encounters for hospice patients entering their 3rd benefit period and beyond to assess terminal prognosis, disease progression, functional decline, and symptom burden. Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions while providing clinical insight on disease trajectories and eligibility criteria. Maintain timely, compliant documentation aligned with Medicare and state regulatory standards, communicating findings effectively to physicians and clinical leadership. Travel within the assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings.
Required Qualifications
- Current and unrestricted Nurse Practitioner license in New Mexico
- National board certification as NP (FNP, AGNP, or similar)
- Minimum 1-year clinical experience
- Solid understanding of hospice eligibility criteria and disease trajectories
- Strong communication, interpersonal, and clinical assessment skills
- Reliable transportation
- Willingness to travel across assigned territory
