Hospice Nurse Practitioner

Job Summary
Conduct comprehensive face-to-face encounters for patients entering or continuing hospice benefit periods, including evaluation, consent, and admission visits to assess eligibility. Provide routine, urgent, or symptom management visits and collaborate with the interdisciplinary team and attending physicians to develop and revise patient plans of care. Educate patients, families, and caregivers regarding disease progression, medications, and end-of-life expectations while documenting all interventions accurately in the electronic medical record. Maintain on-call coverage for evenings and weekends to support continuity of care.
Required Qualifications
- Graduate of an accredited Bachelor of Science in Nursing program and an NP-focused Master's or Doctoral Nursing Program
- Current Registered Nurse and Advanced Practice Nurse license in the state of practice
- Active prescriptive authority and DEA registration as required by state law
- Minimum of two (2) years of experience as a Nurse Practitioner
- Strong knowledge of palliative care, pharmacology, and end-of-life symptom management
- Effective communication and interpersonal skills with patients, families, and interdisciplinary team members
- Reliable transportation, valid driver's license, and current automobile insurance

Desired Qualifications
