Job Summary
Hospice Nurse to provide direct patient care in patients’ homes within Palmdale, Santa Clarita, and Agua Dulce areas under supervision of a registered nurse. Responsibilities include following the plan of care, administering medications and treatments, maintaining accurate patient records, coordinating with the interdisciplinary team, educating patients and families, and ensuring comfort and safety through bedside care, hygiene, nutrition, and contributing to positive patient outcomes. Must demonstrate compassionate, clear communication, strong documentation, and ability to collaborate with healthcare professionals to support end-of-life and comfort-focused care.
Required Qualifications
- Previous experience in nursing or other medical fields
- Familiarity with medical tools and equipment
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Excellent written and verbal communication skills
- Ability to follow plan of care and collaborate with interdisciplinary team
- Maintain up-to-date patient records
- Administer medications and treatments as ordered by physician
- Sterilize and prepare medical tools and equipment
