Hospice Music Therapist

Job Summary
Provide individualized and group music therapy sessions in patients' homes, assisted living communities, skilled nursing facilities, and inpatient hospice settings; assess physical, emotional, cognitive, and spiritual needs to develop personalized interventions; utilize live music, therapeutic singing, improvisation, songwriting, guided imagery, and other evidence-based techniques to address pain and anxiety; collaborate with nurses, social workers, chaplains, and other team members; document assessments and care plans in the EMR in compliance with hospice regulations and HIPAA; educate families and caregivers on using music as a tool for comfort and connection; travel throughout the designated service area; MT-BC credential preferred or required depending on organizational needs.
Required Qualifications
- MT-BC credential preferred or required
- 1-2 years experience as musician/music therapist/related professional preferred
- Experience with serious illness/hospice/palliative care strongly preferred
- Proficiency in voice and at least one instrument (guitar or keyboard)
- Strong interpersonal and communication skills and ability to build trust with patients and families
- Empathy, emotional resilience, sensitivity to diverse backgrounds
- Reliable transportation and willingness to travel throughout the designated service area
