Job Summary

Implement standards of care for hospice patients and families by assessing psychosocial status, evaluating family dynamics, and providing crisis intervention when necessary. Conduct social evaluations to identify risks for suicide, neglect, or abuse, then counsel patients and caregivers on stress management and coping techniques. Educate the interdisciplinary group on cultural needs and assist in preparing advanced directives while participating in regular team meetings to recognize psychosocial impacts on terminal illness symptoms. Maintain accurate clinical notes, coordinate care with physicians and community agencies, and serve as a mandatory reporter for abuse, neglect, or abandonment to the Department of Health. Provide information and referrals for practical needs, support funeral and legal planning, and fulfill on-call duties as defined by policy.

Required Qualifications

  • A graduate of a Master's program in Social Work (MSW) from a school accredited by the Council on Social Work Education
  • Minimum of one (1) year experience in health care
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
  • Possesses and maintains CPR certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer)
  • Prolonged or considerable walking or standing
  • Visual acuity and hearing to perform required social work skills

Desired Qualifications

  • hospice experience preferred
  • Understands hospice philosophy, and issues of death/dying
  • Experience in hospice care preferred
