Hospice Medical Social Worker MSW

Job Summary
Assess psychosocial status of patients and families regarding terminal illness, conduct social evaluations, and provide crisis intervention and counseling. Educate interdisciplinary groups on cultural needs and assist in preparing advanced directives while participating in individualized plan of care development. Maintain accurate clinical notes, communicate with physicians and community agencies, and serve as a liaison for referrals. Report suspected abuse, neglect, or abandonment to state departments and law enforcement immediately. Participate in on-call duties, attend interdisciplinary meetings, and fulfill case assignments while maintaining professional development.
Required Qualifications
- A graduate of a Master's program in Social Work (MSW) from a school accredited by the Council on Social Work Education
- Minimum of one (1) year experience in health care
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Visual acuity and hearing to perform required social work skills
- Prolonged or considerable walking or standing
Desired Qualifications
- hospice experience preferred
- Understands hospice philosophy, and issues of death/dying
- Experience in hospice care preferred
- Demonstrates good verbal and written communication, and organization skills
