Job Summary
Provide compassionate, clinical psychosocial care to patients and families navigating end-of-life illness by completing comprehensive assessments, offering crisis intervention, and supporting advance care planning. Collaborate with nurses, physicians, and interdisciplinary professionals to integrate emotional and practical needs into each patient's plan of care while guiding families through terminal diagnoses. Deliver counseling, resources, and grief support to promote dignity and emotional well-being across the hospice journey. This high-travel position requires daily visits to homes, assisted living facilities, and nursing homes within the Grand Rapids, MI service area, Monday through Friday from 8 a.m. to 5 p.m.
Required Qualifications
- Master's or doctoral degree from a CSWE-accredited school of social work
- Current state licensure as a Licensed Master's Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Minimum 1 year of experience as a licensed Master's Social Worker
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel to homes, facilities, and community settings
Desired Qualifications
- Hospice or palliative care experience
- Clinical experience with grief counseling and end-of-life issues
- Knowledge of Medicare hospice benefit and Conditions of Participation
