Job Summary
Provide compassionate, clinical psychosocial care to patients and families navigating end-of-life illness, including comprehensive psychosocial assessments, crisis intervention, counseling, and advance care planning. Collaborate with nurses, physicians, and interdisciplinary team members to integrate emotional, cultural, and practical needs into patient care plans. Support individuals facing terminal diagnoses by guiding them through emotional, social, and practical challenges while maintaining professional boundaries. Complete daily travel within the service area to homes, assisted living facilities, or nursing homes. Requires current LMSW or LCSW licensure, master's or doctoral degree, and minimum one year of licensed experience. Monday to Friday, 8 a.m. to 5 p.m.
Required Qualifications
- Master's or doctoral degree from a CSWE-accredited school of social work
- Current state licensure as a Licensed Master's Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Minimum 1 year of experience as a licensed Master's Social Worker
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel to homes, facilities, and community settings
Desired Qualifications
- Hospice or palliative care experience
- Clinical experience with grief counseling and end-of-life issues
- Knowledge of Medicare hospice benefit and Conditions of Participation
