HOSPICE MEDICAL SOCIAL WORKER

Job Summary
Provide direct patient care and documentation, conduct clinical assessments for individualized care plans, and apply psycho-social principles to identify factors affecting treatment. Coordinate with agency personnel and other services to optimize patient care; assist with homecare visits; relate well to patients and families at all levels; educate patients/families on utilizing community resources; evaluate bereavement needs and provide bereavement support and grief care through interdisciplinary team coordination.
Required Qualifications
- MSW from a CSWE-accredited program
- minimum one year practical experience in home health/hospice care
- strong interpersonal skills in a multi-disciplinary setting
- ability to travel and accommodate assignments
- minimum of twelve hours of continuing education annually
- background checks/drug-free workplace
