Job Summary
Assess psychosocial status of patients related to illness and environment, communicating findings to the registered nurse. Carry out social evaluations, plan interventions based on findings, and maintain clinical records on all referred patients. Provide information and referral services for patients, families, and caregivers regarding practical and environmental needs while serving as a liaison between them and community agencies. Maintain collaborative relationships with organization personnel, develop contracts with public and private agencies as resources, and participate in in-service education and total plan of care development. Assist physicians and team members in understanding significant social and emotional factors related to health problems and participate in discharge planning.
Required Qualifications
- Graduate of a bachelor's program in social work accredited by the Council on Social Work Education
- Licensed as a licensed clinical social worker by the Board of Behavioral Sciences
- Minimum of one year's experience in health care
- Possesses and maintains current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Demonstrates good verbal and written communication, and organization skills
Desired Qualifications
- Experience in a home health care preferred
