Job Summary
Hospice Social Worker responsible for assessing patients' psychological status related to illness and environment, performing social evaluations, planning interventions, maintaining clinical records, and acting as a liaison between patients/families and community agencies. Responsibilities include participating in the interdisciplinary team, assisting in understanding social/emotional factors affecting care, counseling patients and families on treatment plans and end-of-life care, contributing to the total plan of care and QAPI program, and ensuring regulatory compliance. Requires an MSW or BSW, CPR certification, and Ohio social work licensure, with at least 1 year of healthcare social work experience preferred; full-time at a start-up branch. Required Qualifications
- Has a Master of Social Work (MSW) degree or a Baccalaureate degree (BSW) required from an institution accredited by the Council on Social Work Education
- Possesses and maintains current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- At least 1 year of social work experience in a healthcare setting preferred
- Possesses a current Medical Social Worker Licensure in the states of Ohio
