Job Summary
Hospice LVN/LPN (NOC Shift) provides nursing care to hospice patients and families at home or in a facility, following a plan of care developed through comprehensive assessment. Responsibilities include implementing nursing practice, accurate documentation of patient services to reflect the plan of care, educating patients and families, coordinating hospice services with the interdisciplinary team, maintaining aseptic technique, and managing pain and symptoms. Requires the ability to travel within the designated territory, an active LVN/LPN license in California, BLS certification, and at least one year of LVN/LPN experience. Shift is 8:00pm–8:00am on Saturday, Sunday, and Monday in San Diego, CA; includes hybrid work arrangements per posting.
Required Qualifications
- Current state license as LVN/LPN
- One year of LVN/LPN experience
- Current BLS, hands-on CPR certification
- Valid driver’s license and reliable, insured transportation

- Current LVN/LPN licensure in California
- One year of LVN/LPN experience
