Hospice LPN

Maintain a case load of 18 to 20 patients weekly, providing direct patient care and assessments based on the RN's plan of care. Assess and educate patients, families, and caregivers regarding diagnosis and care plans while managing pain and symptoms. Perform other duties as assigned by the registered nurse. Work Monday through Friday from 8am to 5pm with an average of five patients per day. Cover the Gwinnett, Stone Mountain, Clarkston, Tucker, Chamblee, and Lilburn areas. Requires an LPN license in Georgia, two years of in-home patient care experience, and one year of hospice or palliative care experience preferred. Familiarity with the KanTime EMR system is also preferred.
