Job Summary
Deliver compassionate, patient-centered end-of-life care to patients in their homes within the North Side, PA area, collaborating with an interdisciplinary team focused on comfort and dignity. Conduct ongoing patient assessments for symptom management and condition changes while educating caregivers on relief strategies and the hospice journey. Ensure thorough, compliant EMR documentation and travel to assigned locations, spending approximately 20% of your day driving. Requires an active LPN license, two years of experience, and current driver's license.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state you work
- Two years of LPN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Legally authorized to work in the United States
