Job Summary
Provide direct bedside nursing care to hospice patients and families in home, assisted living, or facility settings under RN supervision. Monitor patient conditions, document changes, and report promptly to the care team. Administer medications and treatments per the plan of care while adhering to infection control and standard precautions. Assist in assessing needs, contributing to individualized care plans, and supporting bereavement activities and closure contacts. Participate in on-call duties and respond to patient needs during scheduled visits or emergencies. May assist with death pronouncements per state law under RN supervision. Collaborate with hospice aides and the interdisciplinary team to ensure comprehensive care delivery.
Required Qualifications
- Graduate of an accredited Practical Nursing program
- Current LPN/LVN license in the state of employment
- Minimum 2 years of LPN experience in hospice, oncology, long-term care, or home health
- CPR certification
- Valid driver's license
- Current automobile insurance
- Understanding of hospice care principles and the unique needs of terminally ill patients and their families
- Excellent patient assessment and clinical skills
- Strong communication and documentation abilities
- Familiarity with regulatory guidelines including Medicare, Medicaid, CMS, and JCAHO standards
- Ability to work independently and as part of an interdisciplinary team
