Job Summary
Provide end-of-life hospice nursing care in patients' homes and communities under RN guidance; implement current nursing practices based on comprehensive assessments and care plans; maintain accurate and timely documentation of patient services; educate patients and families/caregivers; recognize and address changes in a patient's condition; collaborate with interdisciplinary teams (RNs, hospice aides, social workers, and more); participate in Hospice IDT meetings to ensure coordinated care; manage pain and symptoms and evaluate responses to treatments; use and maintain equipment and supplies; contribute to quality improvement and professional development.
Required Qualifications
- Graduate of an accredited practical nurse program
- LPN licensed in the state of employment
- Preferably hospice, palliative or community health home care or medical/surgical experience
