Hospice LPN- 4 days/week

Provide direct, skilled nursing services to hospice patients in accordance with their Plan of Care under RN direction. Build trusting relationships with patients and families while empowering caregivers through education on loved ones' care needs. Collaborate with the care team to deliver comfort, peace, and dignity to those managing advanced illnesses. This role requires an LVN or LPN and involves four days of work per week.
