Job Summary
Provide personal care in patient homes, including bathing, grooming, oral care, dressing, toileting, and repositioning. Assist with transfers, mobility, and safe use of lifts and mobility equipment while observing and reporting changes in condition, skin integrity, intake, and comfort to the assigned RN Case Manager. Document each visit accurately and on time in the electronic record. Support patients and families with a calm, respectful presence, including during the final days of life. This field position requires daily travel between patient homes and independent work without on-site supervision.
Required Qualifications
- Active New Hampshire LNA license in good standing
- Valid driver's license
- Reliable vehicle
- Current auto insurance
- Ability to work independently in the home setting without on-site supervision

- Comfort caring for patients at end of life
- Communicating with their families
