Job Summary
Assist a registered nurse in providing direct patient care and initiate communication with physicians, managers, and health care providers to facilitate optimal care. Instruct primary caregivers, patients, and volunteers in care provision based on the written Plan of Care, while receiving on-site direction from the RN and not altering the patient/family plan. Attend patient conferences to discuss issues and support the team approach addressing practical, social, emotional, and spiritual aspects of care for patients, caregivers, and families. This role includes weekend differential pay and covers Friday through Monday from 3:30pm to 12:00am in Marin and Sonoma Counties.
Required Qualifications
- After-Hours Licensed Vocational Nurse (LVN)
- work under the supervision of a Registered Nurse
- provide skilled patient care services as outlined in the written plan of care
- be available for Friday, Saturday, Sunday & Monday; 3:30pm - 12:00am
- be located in Marin & Sonoma Counties
- Completion of a Licensed Vocational Nursing Education Program from an accredited school
- Minimum of one year of LVN experience
- Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners
Desired Qualifications
- LVN experience preferably in an acute care setting
- Experience in a home health/ hospice setting
