Job Summary
On-call Hospice LPN providing direct care and family support in the home and various nursing facility settings. Responsible for coordinating care, comfort and symptom management, medication management, DME needs, admissions and discharges, proper documentation, and coordination with the hospice interdisciplinary team and facilities. Night shift with on-call weekends; flexible schedule; Dayton, OH. Requires Ohio LPN license, CPR, valid driver’s license; per diem and full-time options; autonomous, self-starter with strong time-management skills.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure by Ohio State Board of Nursing
- CPR certification required
- Holds valid driver's license and current driver's insurance coverage
