Job Summary
Provide skilled nursing care to hospice patients in facilities or homes, perform visits, assist RN and execute care plans; participate in after-hours on-call visits, perform defined procedures under RN supervision, reinforce patient/family education on end-of-life care, and pursue ongoing professional development through training and meetings. Requires LPN license and at least 1 year of clinical experience; Hospice experience preferred; valid driver’s license and reliable transportation for travel within Hamilton County, OH area.
Required Qualifications
- Licensed Practical Nursing Degree
- A minimum of 1 year of Clinical experience/Skilled care
- Hospice experience preferred
- Valid Drivers License and Auto insurance Coverage
- Reliable transportation: Willing to travel
