Job Summary
Provide nursing care to residents and patients, including medication administration, treatment delivery, and documentation under Clinical Director direction. Coordinate care and services with other departments from admission to discharge, assisting Registered Nurses with assessments, data collection, and timely reporting of patient condition changes. Maintain patient and team safety by monitoring for hazards, enforcing infection control policies, and ensuring all physician orders are signed and implemented within specified time frames. Keep work areas clean, sanitary, and efficiently organized while adhering to facility safety rules, regulations, and policies including HIPAA, OSHA, and Universal Precautions. Drive locally as required with a valid license and satisfactory motor vehicle record.
Required Qualifications
- Degree from an accredited school of nursing for the Licensed Practical Nurse program
- Current LPN licensure in the state in which they are working
- Valid Drivers License
- proof of insurance

- Ability to communicate effectively in both written and verbal formats
- Basic computer skills
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
- Weekends only - Saturday & Sunday 8am-4:30pm - 32hrs bi-weekly

- Driver's License Required
