HOSPICE LICENSED PRACTICAL NURSE

Provide direct patient care using a variety of skills and coordinate services while following the plan of care assigned by the Registered Nurse. Complete appropriate documentation as required by policy, procedure, and regulation. Accept and transcribe physician's orders, then forward clinical, progress, and summary notes to the Hospice Registered Nurse for care coordination. Participate in interdisciplinary team meetings and optimize patient care by coordinating with the Registered Nurse, Patient Care Coordinator, and other agency personnel. Ensure effective communication and thorough documentation on a timely basis under Hospice Registered Nurse supervision.
