HOSPICE LICENSED PRACTICAL NURSE

Job Summary
Provides direct patient care in a hospice setting, following the plan of care developed by the Registered Nurse. Coordinates hospice services with the RN, Patient Care Coordinator, and other agency staff to optimize patient care for patients and families. Documents and transcribes physician orders as directed, forwards clinical notes to the RN for coordination, and attends IDT meetings. Requires nursing education from an approved program, current LPN licensure in the branch state (North Carolina), and CPR certification; minimum of one year of clinical nursing experience, preferably in acute care medical/surgical settings; strong organizational, communication, and thorough documentation skills.
Required Qualifications
- Successful completion of nursing education from an approved school of nursing
- Current LPN licensure in the state in which the branch is located
- One year of clinical nursing experience, preferably in an acute care medical/surgical setting
- Ability to independently organize work and provide effective health care services to hospice patients
- Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis
- Effective communication skills (oral and written)
- CPR certification
