Job Summary
Hospice LPN provides direct patient care per patient plan of care, documents care per policy/regulation, coordinates with the RN and team to optimize patient care, accepts physician orders, forwards notes to the RN, and participates in IDT meetings. Requires nursing education and current LPN licensure, plus CPR certification and at least one year of clinical nursing experience. Located in Dunn, NC.
Required Qualifications
- Successful completion of nursing education from an approved school of nursing
- Current LPN licensure in the state in which the branch is located
- One year of clinical nursing experience, preferably in an acute care medical/surgical setting
- Ability to independently organize work and provide effective health care services to hospice patients
- Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis
- Effective communication skills (oral and written)
- CPR certification
