HOSPICE LICENSED PRACTICAL NURSE

Part-time Hospice Licensed Practical Nurse provides direct patient care and coordinates services in a hospice setting. Responsibilities include performing nursing duties, completing documentation per policy and regulations, following the care plan developed by the Registered Nurse, coordinating Hospice services with the RN and care team, accepting physician orders, forwarding clinical notes for coordination of care, and participating in IDT meetings. Requirements: nursing education from an approved school, current LPN licensure in the branch state, one year of clinical nursing experience (preferably in acute care medical/surgical), ability to independently organize work and provide effective patient care, thorough and timely documentation, CPR certification, and strong communication skills.
