HOSPICE LICENSED PRACTICAL NURSE

Job Summary
Hospice LPN provides direct patient care under RN direction, documents per policy, follows the plan of care, transcribes orders, and coordinates hospice services with the RN and care team. Attends IDT meetings and communicates effectively to support patient, family, and providers. Requires current LPN licensure in the state of the branch and at least one year of clinical nursing experience (preferably in acute care medical/surgical settings); CPR certification and strong organizational and documentation skills are essential.
Required Qualifications
- Current LPN licensure in the state in which the branch is located
- one year of clinical nursing experience, preferably in an acute care medical/surgical setting
- Successful completion of nursing education from an approved school of nursing
- Ability to independently organize work and provide effective health care services to hospice patients
- Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis
- Effective communication skills (oral and written)
- CPR certification
